Position Summary:
The CV Surgery Clinic RN supports the daily clinical operations of the ambulatory cardiovascular surgery program to ensure high-quality, efficient, and patient-centered care. This role functions as a clinical resource for providers, staff, and patients, emphasizing care coordination, patient education, and quality improvement throughout the surgical continuum.
Key Responsibilities
Clinical Care & Assessment
- Perform comprehensive patient assessments and document findings in the EMR.
- Execute delegated medical functions in accordance with the Nurse Practice Act, established protocols, and provider direction.
- Review, interpret, and prioritize test results; escalate urgent findings to providers.
- Implement and evaluate individualized plans of care, including post-operative and discharge planning.
- Accurately transcribe and implement provider orders, including verbal orders per policy.
Patient Education & Care Coordination
- Educate patients and families on procedures, treatment plans, medications, and post-operative expectations.
- Provide thorough discharge instructions and reinforce adherence to care plans.
- Communicate test results and assess patient understanding; escalate concerns to providers as needed.
- Coordinate follow-up care, including scheduling tests, procedures, and referrals.
- Manage triage calls, assess symptoms, and direct care appropriately.
Medication & Treatment Management
- Provide medication education, including indications, administration, and potential side effects.
- Process prescription refills per provider protocols and regulatory guidelines.
- Assist in treatment planning and patient self-management strategies.
Clinic Operations & Team Support
- Support clinic workflow and efficiency by assisting with patient flow and in-basket management.
- Collaborate with interdisciplinary team members to ensure coordinated care delivery.
- Delegate tasks appropriately to LPNs and MAs while maintaining RN-level oversight.
- Maintain strict confidentiality of patient and controlled information.
Quality & Safety
- Participate in quality improvement initiatives within the CV Surgery program.
- Ensure compliance with documentation, regulatory, and safety standards.
- Promote a culture of teamwork, accountability, and patient safety.
Minimum Qualifications
- 5–7 years of cardiology experience required.
- Minimum 2 years of Cardiac ICU experience is strongly preferred.
- Strong clinical assessment, critical thinking, and care coordination skills.
- Proficiency in medical and cardiovascular terminology.
- Experience with Epic or similar electronic medical records system preferred.
- Active RN license in the state of MN.
